The form

Regulation 22A of the 2009 Regulations sets the form of notifications. CQC's notifications page links each event to its own form and guidance. A reportable incidents policy should name who sends the notification, by when, and where the copy is filed. That is the "person responsible" CQC asks for on every document.

Who this is not for. Providers who need advice on a specific incident now: contact CQC directly. This guide lists the categories, not how to judge a case.
